Background: Understanding an athlete’s workload is one way to determine the likelihood of receiving a sports injury. Workload variables are categorized as either internal load (IL) such as heart rate, or external load (EL) which include speed, distance or volume. Objective: This study investigated the correlation between IL and EL measured by micro-technology in female college soccer players. In addition, the utility of IL and EL to predict risk of soft tissue injury on lower limbs was examined. Method: 23 NCAA Division One women soccer players 19.2 ± 1.2 years old, 168.2 ± 7.3 cm, and 141.0 ± 17.9 kg were recruited. Only field players with no prior lower limb injuries were included. IL measurements collected were ratings of perceived exertion (S-RPE), average heart rate (Avg-HR), training impulse (TRIMP i.e., HR x time) and estimated maximum heart rate (Max HR). Total distance (TD), average speed (Avg-Spd), high speed running distance (HSR), estimated maximum speed (Max speed) and intensity volume index (VI index) were identified as EL. The workload data were categorized as being either acute or chronic. Acute was defined as the measured average workload the seven days immediately prior to the injury, while chronic workload meant the average workload 21 days before the athletes were hurt. Spearman correlation was used to examine the relationships between IL and EL and one-way ANOVA and Kruskal Wallis tests were conducted to investigate the mean differences between injury groups. Results: There were significant positive correlations between S-RPE and TD (r = .82, p < .001), TRIMP and TD (r =.75, p < .001), Avg-HR and Avg-Spd (r = .80, p < .001), and H-HR zone and HSR (r = .60, p < .001). The results indicated that the acute Avg-HR, the A/C ratio of Avg-Spd and VI index were significantly (p = .001) higher in the injured compared to the non-injured group. Conclusion: The study indicated that internal and external load were significantly correlated in this group of female soccer players. Also, acute Avg-HR and A/C of speed and volume index may predict the risk of soft-tissue injury in female athletes.

Obesity has developed into a worldwide health problem that is associated with many risks. The elements causing obesity are complex and numerous including behavioral, psychological, and physiological. Traditional methods of weight loss have demonstrated short-lived positive health benefits and minimal long-term weight loss, which has led to the prevalence of bariatric surgery as an answer to long-term weight loss for Class III obesity. Gastric bypass surgery has become especially popular for its numerous benefits including successful weight loss, improvements in obesity-related diseases, and increased lifespan. Bariatric surgery is still not a perfect solution. Negative effects after surgery range from surgical complications and vitamin deficiencies to altered hormonal levels and metabolic rates. Many questions regarding bariatric surgery still remain including the impact of adolescent bariatric surgery, long-term bone effects, and long-term psychosocial and lifestyle components of bariatric patients. Understanding the good, the bad, and several of the remaining questions regarding bariatric surgery, will help health professionals be more appreciative of the complexity of treating their obese patients.

Weight cycling (WC) is characterized by repeated bouts of weight loss followed by regain. WC has been associated with a number of adverse health consequences and is a risk factor for cardiovascular disease. Body weight regulation is complex. Little is known about why women who intentionally lose weight are so likely to regain their weight back. Humans are motivated by a variety of psychological pressures as well as physiological stimuli that influence eating behaviors and weight control. One of the complex factors that has been shown to predict weight regain, in weight-reduced individuals, is hunger. Ghrelin is a known gastrointestinal hormone that rises during weight loss and is a strong trigger of hunger and increased appetite. Increased ghrelin levels have been associated with disordered eating behaviors and active weight loss. The Three Factor Eating Questionnaire (TFEQ-R18) describes elements that may affect hunger and satiety. These factors are: cognitive restraint (CR, defined as regulating food intake because of weight maintenance), uncontrolled eating (UE, defined as difficulty in regulating eating), and emotional eating (EE, refers to the tendency to eat more than needed because of mood state). Objective: The purpose of this study was to explore the associations of fasting plasma ghrelin with eating behaviors and weight cycling in overweight and obese women. Methods: This is a cross-sectional observation of women aged 20-60 years who completed a Weight and Lifestyle Inventory (WALI) and the TFEQ-R18. Women provided a 12-h fasting blood sample and plasma ghrelin was measured using a commercial radioimmunoassay (ELISA kit Cat# EZGRA-88k). Intra- and inter-assay CVs were 88.4% + 13.8% and 84.4% + 8.4% respectively. Descriptive data were computed and Pearson correlations were assessed adjusting for age and body weight (SPSS, v23). Results: A WC Index (WCI) was computed as number of WC reported x the amount of weight lost per cycle. 61 women (mean age: 39.3 + 11 yr; BMI: 31.4 + 7; WCI: 70 + 60; range = 0 to 253) completed questionnaires. Ghrelin was significantly and negatively correlated to weight (R= -0.25, P = 0.03), BMI (R= -0.32, P = .006), UE (R = -0.29, p = 0.02), and EE (R = -0.29, p = 0.04). Ghrelin was not significantly related to WCI. WCI was not significantly correlated with any TFEQ-18 subscales. Conclusion: In this observational study, lower ghrelin was associated with higher UE and EE. Thus physiological hunger sensations from ghrelin secretion, is not a likely stimulus of eating behavior in these women. There are a host of psychological triggers, such as stress, loneliness, guilt, anger etc. that may enhance eating. Future research will need to explore what psychological triggers influence eating behavior and why obese women are resistant to the powerful physiological hunger cues of ghrelin.

Sustainability, as it relates to nutrition, affects all aspects of food from systems-level production to consumption. Viability of local food systems in the southwest of the United States has been largely understudied. In order to address this gap in the literature, semi-structured interviews were conducted with 20 farmers in Arizona and New Mexico to determine best practices, challenges and barriers to farming. Interviews were recorded, transcribed, and coded for themes. Many trends were consistent with those reported elsewhere in the US, but the importance of water emerged, a unique need not explicitly noted in other regional studies.
Vegetarian diets are typically more sustainable than omnivorous ones due to using less environmental resources in the production of food. An important consideration with plant protein and vegetarian diets, however, is whether this would affect athletic performance. To examine this, 70 male and female endurance athletes were compared for maximal oxygen uptake (VO2 max), peak torque when doing leg extensions, and body composition. Vegetarians had higher VO2 max, but peak torque was not significantly different by diet. Omnivores had higher total body mass, lean body mass, and there was a trend for peak torque to be higher.
To investigate whether plant-protein can comparably support development of lean body mass and strength development in conjunction with strength training, 61 healthy young males and females began a 12-week training and protein supplementation study. While previous training studies have shown no differences for lean body mass or strength development when consuming either soy (plant) or whey (animal) protein supplements in very large amounts (>48 grams), when consuming around 15-20 grams, whey has contributed to greater lean body mass accrual, although strength increases remain similar. The present study matched supplements by leucine content instead of by total protein amount since leucine has been shown to be a key stimulator of muscle protein synthesis and is more concentrated in animal protein. There were no significant differences between the whey or soy group for lean body mass or strength development, as assessed using isokinetic dynamometry doing leg extensions and flexions.

Having accurate measurements of sedentary behaviors is important to understand relationships between sedentary behaviors and health outcomes and to evaluate changes in interventions and health promotion programs designed to reduce sedentary behaviors. This dissertation included three projects that examined measurement properties of wearable monitors used to measure sedentary behaviors. Project one examined the validity of three monitors: the ActiGraph GT3X+, activPAL™, and SenseWear 2. None of the monitors were equivalent with the criterion measure of oxygen uptake to estimate the energy cost of sedentary and light-intensity activities. The ActivPAL™ had the best accuracy as compared with the other monitors. In project two, the accuracy of ActiGraph GT3X+and GENEActiv cut-points used to assess sedentary behavior were compared with direct observation during free-living conditions. New vector magnitude cut-points also were developed to classify time spent in sedentary- and stationary behaviors during free-living conditions. The cut-points tested had modest overall accuracy to classify sedentary time as compared to direct observation. New ActiGraph 1-minute vector cut-points increased overall accuracy for classifying sedentary time. Project 3 examined the accuracy of the sedentary sphere by testing various arm elevation- and movement-count configurations using GENEActiv and ActiGraph GT3X+ data obtained during free-living conditions. None of the configurations were equivalent to the criterion measure of direct observation. The best configuration of the GENEActiv was: worn on the dominant wrist at 15 degrees below the horizontal plane with a cut-point <489 for each 15-second interval. The best configuration for the ActiGraph was: worn on the non-dominant wrist at 5° below the horizontal plane with a cut-point of <489 counts for each 15-second interval. Collectively, these findings indicate that the wearable monitors and methods examined in this study are limited in their ability to assess sedentary behaviors and light intensity physical activity. Additional research is needed to further understand the scope and limitations of wearable monitors and methods used to assess sedentary behaviors and light intensity physical activity.

It is widely documented and accepted that athletes have difficulty maintaining adequate hydration status and that dehydration is a key risk factor for the heat-related illnesses commonly observed among athletes. Research has also suggested that hydration status can influence cognitive performance. Educational interventions focused on rehydration strategies have had minimal success reducing dehydration rates; hence, alternative interventions promoting adequate hydration status in athletes should be explored. This trial examined the efficacy of a commercial hydration mobile application (app) for reducing dehydration rates in campus athletes. Fifty-eight college students aged 18-40 y, who participated in club-level collegiate athletics were recruited from a large Southwestern university and randomized by team to one of two study arms, the Standard of Care – Education (EDU) or the hydration mobile app (APP), to determine if app technology improved hydration status as compared to traditional education messaging. Twenty-three (79%) in the EDU group and twenty (69%) in the APP group were mildly-dehydrated at baseline based on the three-day averages of hydration assessment (USG 1.010). Moreover, 31% (n=9) and 28% (n=8) of the EDU and APP groups, respectively, were dehydrated (USG 1.020). No significant differences were found between the EDU and APP groups following the intervention. Three-day average post-intervention USG testing showed 76% (n=22) and 72% (n=21) of the EDU and APP groups respectively were at best mildly-dehydrated. Additionally, 28% (n=8) and 17% (n=5) were considered dehydrated. Neither intervention improved hydration status after four weeks of treatment. Further analyses of cognitive measures were conducted by hydration assessment groups at baseline and post-intervention: hydrated (HYD) (USG < 1.020) or dehydrated (DEH) (USG 1.020). No significant differences between hydration status were found between intervention groups. Additionally, no significant improvements were seen for either group, which indicates there is still a need for a novel way to improve hydration status in this population. Multi-dimensional interventions and individualized interventions to improve hydration status in this at-risk population may be more effective. Additional research should be conducted to determine if there is any cognitive performance enhancement associated with dehydration or mild-dehydration by reassessing previous data and conducting future trials.

Although maintaining an optimal level of muscle quality in older persons is necessary to prevent falls and disability, there has been limited research on muscle quality across age and gender groups. The associations of muscle quality, muscle strength, and muscle mass also remain less explored. Purpose: This study examined the muscle quality differences (arm and leg) between healthy young and elderly adults across gender groups. This study also examined the associations of muscle quality, muscle strength, and muscle mass in young and elderly adults, respectively. Methods: Seventy-one total subjects were recruited for this study within age groups 20-29 years old (20 females and 20 males) and 60-80 years old (18 females and 13 males). All participants completed anthropometric measures, dual-energy x-ray absorptiometry, pulse wave velocity, handgrip strength and leg strength tests, gait speed, and sit to stand test. Results: Young male adults had a greater leg muscle quality index (leg MQI) than did elderly male adults (21.8 Nm/kg vs. 16.3 Nm/kg, p = 0.001). Similarly, young female adults had a greater leg MQI than did old female adults (21.3 Nm/kg and 15.6 Nm/kg, p<0.001). For arm muscle quality index (arm MQI), there was a gender difference in young adults (p = 0.001), but not for the elderly adults. Among elderly adults, there was a positive association between leg MQI and isometric leg strength (r = 0.79, p<0.001). Notably, there was a negative association between leg MQI and leg lean mass (r = -0.70, p<0.001) and between arm MQI and arm lean mass (r = -0.58, p = 0.001). In young adults, there was also a positive association between arm MQI and handgrip strength (r = 0.53, p<0.001) and between leg MQI and isometric leg strength (r = 0.81, p<0.001). There was no association between muscle quality and muscle mass in young adults. Conclusion: Young adults had a greater leg muscle quality than did elderly adults in both men and women. Leg muscle quality is positively associated with leg muscle strength in both young and elderly adults but is inversely associated with leg muscle mass in the elderly adults.

Currently, there has been limited research on evaluating the social media use and
competency level of registered dietitian utritionists (RD/N). With health information increasingly sought on social media, it is imperative to understand the social media competency of health professionals. The social media use, reach, and competency level of a nationwide RD/N sample was assessed utilizing an online survey. The sample (n=500) while mostly female (97%) was representative of RD/Ns compared to the nationwide statistics from the Commission on Dietetic Registration. The sample included RD/Ns from forty-six states with California (n=44), New York (n=42), and Texas (n=34) having the largest proportion of respondents. The majority of RD/Ns engage in social media for personal use (92.4%) and 39.2% engage for professional use. One hundred and twenty-five RD/Ns reported 777 ± 1063 (mean ± SD) social media followers. As compared to non-millennial RD/Ns, millennial RD/Ns engaged significantly more in social media for personal and professional use (+10% and +13.5% respectively, p<0.001) and scored significantly higher for social media competency (p<0.001). Additionally, food and nutrition management and consultant/private practice/industry RD/Ns had significantly higher competency scores than clinical RD/Ns (p=0.015 and p=0.046, respectively). RD/Ns who use social media personally and professionally had a significantly higher competency score than RD/Ns who did not (p<0.001). There were significant associations of Facebook, Twitter, total followers and total average followers with the social media competency score (r=0.265, 0.404, 0.338, & 0.320, respectively) in RD/Ns. Specifically, the social media competency score, was found to explain 16% of the variation in the number of Twitter followers and 10% of the variation in the average number of followers by platform. These data suggest an opportunity to increase RD/Ns’ social media reach (i.e. following) by improving competency level.

Background: The quantification of the relationships between walking and health requires that walking is measured accurately. We correlated different measures of step accumulation to body size, overall physical activity level, and glucose regulation.
Methods: Participants were 25 men and 25 women American Indians without diabetes (Age: 20-34 years) in Phoenix, Arizona, USA. We assessed steps/day during 7 days of free living, simultaneously with three different monitors (Accusplit-AX120, MTI-ActiGraph, and Dynastream-AMP). We assessed total physical activity during free-living with doubly labeled water combined with resting metabolic rate measured by expired gas indirect calorimetry. Glucose tolerance was determined during an oral glucose tolerance test.
Findings: Based on observed counts in the laboratory, the AMP was the most accurate device, followed by the MTI and the AX120, respectively. The estimated energy cost of 1000 steps per day was lower in the AX120 than the MTI or AMP. The correlation between AX120-assessed steps/day and waist circumference was significantly higher than the correlation between AMP steps and waist circumference. The difference in steps per day between the AX120 and both the AMP and the MTI were significantly related to waist circumference.
Interpretation: Between-monitor differences in step counts influence the observed relationship between walking and obesity-related traits.

Background:
We describe the study design and methods used in a 9-month pedometer-based worksite intervention called “ASUKI Step” conducted at the Karolinska Institutet (KI) in Stockholm, Sweden and Arizona State University (ASU) in the greater Phoenix area, Arizona.
Methods/Design:
“ASUKI Step” was based on the theory of social support and a quasi-experimental design was used for evaluation. Participants included 2,118 faculty, staff, and graduate students from ASU (n = 712) and KI (n = 1,406) who participated in teams of 3–4 persons. The intervention required participants to accumulate 10,000 steps each day for six months, with a 3-month follow-up period. Steps were recorded onto a study-specific website. Participants completed a website-delivered questionnaire four times to identify socio-demographic, health, psychosocial and environmental correlates of study participation. One person from each team at each university location was randomly selected to complete physical fitness testing to determine their anthropometric and cardiovascular health and to wear an accelerometer for one week. Study aims were: 1) to have a minimum of 400 employee participants from each university site reach a level of 10, 000 steps per day on at least 100 days (3.5 months) during the trial period; 2) to have 70% of the employee participants from each university site maintain two or fewer inactive days per week, defined as a level of less than 3,000 steps per day; 3) to describe the socio-demographic, psychosocial, environmental and health-related determinants of success in the intervention; and 4) to evaluate the effects of a pedometer-based walking intervention in a university setting on changes in self-perceived health and stress level, sleep patterns, anthropometric measures and fitness. Incentives were given for compliance to the study protocol that included weekly raffles for participation prizes and a grand finale trip to Arizona or Sweden for teams with most days over 10,000 steps.
Discussion:
“ASUKI Step” is designed to increase the number of days employees walk 10,000 steps and to reduce the number of days employees spend being inactive. The study also evaluates the intra- and interpersonal determinants for success in the intervention and in a sub-sample of the study, changes in physical fitness and body composition during the study.
